These forms are available in PDF format
These forms can be printed off and filled in by hand (in blue or black pen) or completed onscreen and then printed.
Note that the information sheets - forms G7, CYPF4 and PPPR14 - must be filed with the court on yellow paper, under r20(1)(b) Family Courts Rules 2002. They may be printed on yellow paper, or photocopied on to yellow paper. This is to make information sheets easily visible in the file for the Judge, or for a member of the court staff, who may need to refer to it frequently and to find it quickly. As it is required by the Rules, the papers will not be accepted for filing if the information sheet is not on yellow paper.
If you complete the form by hand, it must be clearly legible. If it is not, the court will not accept it for filing.
You should complete all the relevant parts of the forms.
Only a limited amount of information can be filled in on the forms. If you run out room and need to write or type more, please continue on a blank sheet of paper and attach this to your application.
In the Family Court field, you can hold the mouse on the up or down arrows to scroll through all the Family Court locations in New Zealand.
Where there is a "check box" click on the box that appropriately describes your situation.
Forms that require the applicant to swear or affirm need to be signed by you in the presence of a lawyer (not your own) ), a Justice of the Peace, or a Court Registrar.
All forms must be signed and dated. Forms completed on screen must be printed before being signed in the usual way.
Some forms have parts which will be completed by the Court registry, e.g. the case number ( FAM...), and the information about setting the case down for a hearing.
